GENERAL TERMS AND CONDITIONS

If you decide to schedule a follow-up appointment after our first conversation, a written treatment agreement will be signed. This agreement will include clear arrangements regarding privacy.

The agreement ends when:

We mutually agree that no further sessions are needed

You choose to stop the sessions

During the sessions, it becomes clear that the issue you are seeking help for, requires more than I, as a therapist, can provide. In that case, I will, if you wish, refer you to a more appropriate form of therapy

When agreeing, a fee per session will be agreed upon. This is recorded in the treatment agreement.

Rescheduling or canceling an appointment is free of charge up to 24 hours before the scheduled time. Appointments canceled less than 24 hours in advance will be charged for half the session fee. If you are absent without a cancellation, you will be charged the full amount.

During treatment, a record of notes from the sessions and other relevant documents will be kept. These can be reviewed upon your request. Maintaining a file is a legal obligation under the WGBO (Dutch Medical Treatment Agreement Act).

There is a professional confidentiality agreement in place for De Lijfpraktijk. Information will only be shared with third parties (doctors and other therapists) with your explicit consent.

My practice is not part of the regular mental health care system. If you are currently seeing a psychiatrist, exploring your options with your GP, or engaged in another treatment path, I am open to collaborating with other practitioners/coachers/caregivers. A trajectory with me can stand entirely on its own, independent of what you are doing or have done before, or it can be developed in coordination with other support. This is always done in consultation with you and with your consent. ​

De Lijfpraktijk is not liable for any negative consequences arising from the provision of incorrect or incomplete information.

De Lijfpraktijk is not liable for damage, theft, or loss of property, or accidents in and around the practice.

COMPLAINT PROCEDURE
Do you have a complaint? It is important that you let me knwo, so we can iscuss it together as soon as possible. If we are unable to resolve it, you can submit a complaint to the independent complaints committee of my professional association (cooperating with S.C.A.G.). You can find all the necessary information for that step here.
